How to prepare your health records for a doctor visit
By HealthArt · Published
Your appointment is easier to prepare for when the right records are ready to find. Instead of searching through email during the visit, collect the relevant documents and make a short list of what you want to discuss.
You don’t need a perfect archive. Start with what matters for this appointment.
1. Gather the relevant documents
Check any instructions from the clinic. Depending on the appointment, your records folder might include:
- Recent lab reports and earlier results for comparison
- Prescriptions and consultation notes
- Hospital discharge summaries
- Imaging reports and any image files the clinic has requested
- A referral letter, if applicable
Keep full documents rather than cropped screenshots. Confirm that every file belongs to the right person.
2. Make a short timeline
Create a simple note alongside the documents:
- Date of a test, consultation or hospital stay
- Document or report name
- Where the original file is stored
Use this as an index, not a replacement for the records themselves. Link an entry to its document so you can open the original quickly.
3. Prepare your questions and medicine list
Write down your main questions, symptoms you want to discuss and the medicines you take, including vitamins and supplements. NHS guidance recommends preparing these notes before an appointment. Read the NHS appointment checklist.
Keep this note separate from any automatically extracted report results so it is clear which information you added yourself.
4. Check access before the visit
Open your files on the device you plan to bring. If the clinic accepts shared links, check that the intended person can access them and whether a PIN or expiry applies.
Ask the clinic whether it needs files in advance, printed copies or a particular format. If internet access could be unreliable, keep a secure offline copy of the essential documents.
Share only the relevant person’s records—not a folder containing everyone’s health information.
5. Keep the next steps with your records
During the visit, ask about anything you do not understand. Before leaving, clarify what happens next and when. Afterwards, save your notes and any new documents together. The NHS checklist includes questions about follow-up.
Before you leave home
- Relevant original reports are collected
- Files belong to the correct person
- Questions and medicine list are ready
- Documents open on your device
- Sharing and any clinic instructions are checked
